If using a tablet or mobile device, it will need to have a camera to take a picture of your void check or letter from bank. If using a computer, please have your document scanned and ready to upload.

Teens Ages 13-17 – Requires notary, please print PDF form and sign in front of a notary.

Children Ages 0-12 with one parent/guardian on file
